how do you set up FormMail on new domain?

how do you set up FormMail on a new domain?

I have gotten this far in my coding but im stuck on the form action part...

can someone please help me out?
there should be some sort of "http://sonic.net/formmail.php" thing i need to put in there but i dont know what it is...

According to that, you'll need to use

/cgi-bin/formmail.pl

as the action for your form.
