Yeah I'm already using the native app. I've been experimenting with different queues on the WAN port of my router and I think it's probably related to that. For whatever reason whatever inbuilt congestion control mechanisms don't seem to be working properly for upload, or they are too severe resulting in the lower upload speeds. I'm going to continue fiddling. Does Sonic provided hardware have specific configuration for talking to the ONT?
Looking at the spec sheet for the
Adtran 622v it boasts "Hardware Priority Queues on the Downstream Direction in
Support of CoS" I wonder if that's why downstream is different to upstream?

Yeah I had an ONT update but the speeds were the same before/after, when I went on to explain that I was only getting ~5-6Gps up rather than 8.1 (which I'm getting for download) they said "... It sounds like you are getting very good speeds. If you run into trouble with your connection again, please do not hesitate to reach out. Thanks!" and they closed the ticket.
So I guess symmetric speeds aren't really expected, despite being advertised. Or they just don't want to spend time supporting a 3rd party router. I tried the restricting upload trick and it seems to at least result in more consistent upload speeds, but they are still below expected.
I have a few other ideas I'm going to try out when I can get the stuff I need for it, so if I find out anything useful I'll post back here.

I would not recommend running any QOS on your router, there's really no point unless you find that you're saturating the link and causing degraded performance for sensitive applications (which seems unlikely) and will likely only end up reducing performance. It doesn't look like you mention what OS/Hardware you're using to test but do mention it is the same if directly connected to the ONT or your UDM. Have you tried with more than one client device and/or made any tuning changes? I've heard some anecdotes that Windows default TCP/IP settings can negatively impact 10G performance so that might be a place to look as well along with the standard issues with drives, transmit and receive offloading, etc.

So I decided to test this a bit more scientifically. I ran 10 speedtests for each configuration to see if restricting the egress rate of the network interface on my router that is connected to the ONT made any difference (spoiler alert: it did)
My router is a Mikrotik CCR2116-12G-4S+ and I alternated between each configuration to rule out any general congestion-in-the-moment issues.
Restricted = egress rate restricted to 7.5G
Unrestricted = no restriction
