on-line email form

I'm trying to create a form, on a webpage from my website, hosted by sonic ,that allows a viewer to email me a message in a textarea box.
What is the .php address I use to have the form sent to my info@name_ of_my_website.com ?
Places,websites, I can go to see how the code for this form should look would also be helpful, thanks
