access to mySQL db broken

http://www.ochslabs.com has on its home page a "Provider Login" button, which gives access to our Customer DataBase (CDB), a mySQL db. That login no longer works, probably because our failing programmer inadvertently altered some of code.

The path is http://www.sonic.net/lochs/~lochs/ochsl ... plogin.php. When I push the "Provider Login" button I get "no input file." Yet I can look at the PHP of plogin.php and it looks as it should.

Can someone help me? I'm at 707-396-3598. I need someone handy with mySQL as well.

I noticed that you had a superfluous "lochs" directory in the url, so I tried http://ochslabs.com/cdb/prov/plogin.php

The PHP errors there show that it's trying to find a file at "/nfs/WWW_pages/lochs/ochslabs.com/cdb/prov//prov/mdlib.php" which appears to have a superfluous "prov" directory in the path.

Perhaps addressing that would help?
