Default area code prefix

I have a very strange problem with my voice service. When I try to call any number that is not in my area code, the phone service prefixes my area code to the number and truncates last 3 digits. For example: If I am trying to call 415 123 4567 and my area code is 212, the number 212 415 1234 is dialed instead. It is very bizarre. Has anyone experienced a similar problem before? I tried changing every possible setting on phone (Motorola L603M), but to no avail. Thanks!
