UI bug: fax header relies on hidden fields

When you choose to send a fax with a cover letter (by checking the 'Include cover letter' box), you can specify the "From" name and the "From" fax number, and these are used in both the cover letter and the fax header that appears at the top of each page.

However, if you uncheck the 'include cover letter' box, these fields are hidden, even though they are still used to populate the fax header.

I like that the form changes dynamically when the box is checked/unchecked, but I think that the "From name" and "From fax number" fields should be visible when the box is unchecked.

Hi Tom,
Thanks for your feedback. We've fixed the Send A Fax tool so that the default values will be used if the cover letter is unchecked. In the interest of keeping the UI as minimal as possible, we'll likely keep the "from" fields in the cover letter section. The tool tries to be intelligent about the defaults; it uses the account holder's name, and the existing FaxLine number on the account if there is one, or the fax number specified in the account information, if present.
